how it works? do I need to buy two? one goea do cable box and the other to the tv?

What is the cable box model number? You only nee one to plug into the IR IN port on the back of the box. The receiver is placed on or near the TV frame

Will this emitter work with an Xantech connecting block?

It will not work with Xantech. They have a different pin out and the block also requires 12v as ours is 5v. We do offer our IRP repeater kits if interested.

Will this work with Cisco explorer 8640hdc?

when plugged into the IR port on the Cisco Explorer 8640hdc will the the IR on the front of the 8640 also still work?

States it works with 9865hdc but is not. is this supposed to?

It should work. Try unplugging the power from the box for 30 seconds while keeping the IR receiver plugged in. The box will reboot.
