Is there a detachable bottom? What part lifts so that you can insert Kleenex?

There's an inside box with inserts for your pictures. You simply pull off the outside box to insert your tissue box and pictures.

Is the box heavy enough that when you pull out a tissue the box does not move?

As the other reviewers said, it is heavy enough that it doesn't move. However, you will find when a new box is inserted, tissues do need to be tugged on a little. I just gave one to my sister but rather than use photos, I used cardboard from a tissue box for the design. One could use any type of decorative paper and glue to card stock for stability. Enjoy!

How do you open it to insert pictures ? It looks like the bottom comes out. Is that right?

Yes! Just pull the top portion off by gripping on to the hole where the tissue comes out. You can then slide your pictures on the sides of the cube and drop the box of tissue in the middle. Then just replace the top portion and you're good to go!

How do you recommend customizing the photo size if I do not have a printer at home? Is there a store that will print custom sized photos?

This item Holds two 4-1/2x 5-3/8 inserts and two 4-3/4x 5-3/8 inserts. I would recommend using 5x7 photos and cutting them down to fit. That would be the easiest thing to do.
