A Piece of Mind to Reclaim Your Time
I bought this book physically and on Kindle. Why, because I needed immediate access to help write my notes when I was mentally spent or just a lot of clients that at times seem to run together. I gave it 4 because I thought it needed a few more templates to really make it a 5. But, the author is right on point in many of her observations, recommendations, and or suggestions. If you are proficient already at writing notes and have been in the field for a while this probably is elementary to you, but I have to think at times even you may be a little dusty and need a little something to help you clear out the cob wells. Even with a few templates, her thought prodding of "Who, What, When, Why, and How," helps the mind put things in order.

Informative
This book was very informative and concise. The information provided me with direction, insight, and clinical guidance for writing effective notes. I would recommend this book to other clinicians. My only critique is that the formatting of the book could've been better. The book does not include a table of content which would have been helpful when navigating through the book. Having a table of content would've provided the book with structure and would've been more professionally polished. Overall, this is a good book.

Easy to Follow
Easy to follow as well as implement. The example write ups were very useful in guiding note writing. DAP or DIRP much easier than SOAP to follow.

Only 16 pages of information and that’s 16 too many
I am always looking for ways to improve my progress notes and thought this looked promising. The description lists 25 pages (yes, that in itself is too short) but it’s actually only 16 pages of information. The rest is just the author sharing useless tips and information. There are very few examples. Literally just two for individual, group, and case management. Only uses DAP format. I was looking for good interventions to use but there are none. It’s just [insert intervention here]. This should have cost $1, not $6.99. The author seems to think very highly of herself but I question her experience and qualifications after reading this. One star is too much. Spend your money on quality content.

honestly… she’s real.
I’ve worked in addiction Tx since 2005. Every position from clinical to training to supervisor.. and she’s written real notes in here.Very helpful to the new staff, like CRS’s, new therapists, out of school people.Thanks and honestly, great idea - hat’s off to you.

Disappointing
Frankly it’s less than I expected. It would be helpful if the author provided access to the templates that we could automatically download and adjust to our own needs. They also Seem to be more appropriate two clinical practices rather than the individual practitioner.

This book had a sufficient number of examples that would keep your supervisors happy.

Consize and well written
Helpful guide to writing progress notes

Does not amount to a useful resource
This publication's author mentions sending clinical notes back to her supervisees for revision if they contain typos. However, from the first page onward, her book is full of typos and grammatical oversights. The publication's length is more of a handout or booklet, so doesn't justify the price. The content is minimally helpful: a basic, proposed outline for an agency-oriented clinical records template is introduced, with no analysis, followed by various samples of its application. I wouldn't recommend this particular source in its current edition.
