I have used these indoor only. They work pretty good.

Can these be used in an enclosed fixture?

Yes. I have four enclosed floodlights in backyard. They have been working great for a year now without any issues. Lights are on timer and are on several hours each night.

Installed 4 bulbs in a ceiling fan and one bulb stays on at very low setting. Any ideas as to why?

I responded earlier that I don't know. That is true. It sounds like it might be a wiring issue in the fan. I installed my bulbs in fixtures at my door and my garage, and they work perfectly.
