Does this camera come with all items needed to operate it or do i need something else?

The extention cable is only needed for longer vehicles. The extension cable doesn't come with a camera. When you purchase a camera you will also want to but a memory card

I need to get 50 foot will the signal be strong enough for this?

The cable is very flimsy so I suspect its not shielded to some degree. Although its hard to determine whether this cable length will be able to provide enough current for the remote camera even if I new what device you were getting the video back to. You may want to ask the vendor of your camera device what wattage if any is annotated in the documentation of your primary device. 16.5ft extension was plenty to the rear my tailgate on a f150 with distance to spare.
