Q: What door thickness does this require?

I really can't answer this, we used for the sides of vanity. There is a heat duck that goes right into the vanity, so we could get heat out. Maybe 1/2" ??

On one complete awenta ventilation grille was include but the 4 pack I think you are talking about fits it thicker dimension for for a regular interior door not a bifold door

Q: What is the minimum and the maximum thickness of door, that this will work for?

They are made of plastic. I shaved down the collar and vents to make them work on a thinner hollow core bedroom door.
