Q: If I used this tool on cardstock (60 lb), would it poke holes all the way through the paper, or just make indentations? Looking for 5 holes per inch.

It might punch holes in paper if you had the paper on a spongy surface. It mainly makes indentations to indicate where to punch holes.

Q: Can it poke holes or just mark borders?

No it will not punch holes. It only marked the spaces. It also can be used for decorative stitch marks.

Q: does it come with a handle

Q: How deep are the cuts (I need to perforate upto maximum 2mm depth)?

I believe that depends of the pressure you use, but in fact this tool is made for give a guide or to make marks on the surface, not to cut
