Q: Can the child see clearly of the eye patches?

Not really. We ended up just taking the costume off.

Q: What size does a small fit

I believe it should fit a 4/5 but am not completely certain.

Q: Can the patches be removed?

The one we bought had no patches, only the eyes and mouth!

Q: Will this fit a child who is tall for their age? 5'4?

No. It fit my nephew who was like 4ft tall and it didn't come to ground. I think if you don't mind it being short. And it was really hard to see. We had to put hat on so eyes would stay in place.
