Q: What are the dimensions of the transmitter? Also, how do you set up the system? Does it require someone coming to so it?

The transmitter is smaller than a credit card, not very big at all. Setup is pretty easy. One of our maintenance men does it. Only takes 10 or 15 minutes. The system works well.

Q: will it call a cell phone??

It will call any number that you put into it, cell phones included.

Q: Will it work in a three story house from attic to basement if placed on the first floor.?

It should, but just to be safe, you probably want it as centered in the home as you can, so that you will have full coverage. My Mom has a basement, first and second floor. I installed it on the first floor in the center of her house, and tested it in every room, as well as going outside in the back and front yards, and it worked.
