Do I need the accumulator and strainer for this unit?

I think both would be recommended for an RV or boat install. It will run without, but an accumulator will smooth out water flow for showers. A strainer is always recommended.

Can you adjust the pressure switch?

Yes you can. Remove the center phillips head screw on the end cap, remove the cap and the adjustment screw is the middle one between the two wire connections. Turn clockwise a quarter turn at a time till surging stops.
