What is the packaging like? Does it come in a nice box?

Mine did not but they were undamaged from shipment

Should I order for my first name or last?

Because it is a single initial monogram, either first initial or last initial is acceptable. In the case where multiple initials are used, in 2 initial monos, it's FL, in the case of all 3 initials: either FML or (more formally), FLM is the appropriate format. I ordered these using my first initial.
