How many washers are needed?

Hi, 3 rubber washers are needed. They are pre-installed in the bracket and hose. Please refer to the installation video above.

Which part has the O-Ring to be removed for more pressure?

The O-ring is inside the small plastic bag. You decide to put it or not on the shower head, before is attached to the hose. I tried without the ring first, and the pressure was painful! :) I decided to install the O-ring and the pressure is also very good.

I want to connect this to a shower extension arm, is that possible?

I believe you can install any shower head to an extension arm.

Is the shower head holder/connecting bracket, including the ball joint, all metal?

Yes it came with all mounting hardware. Their claim as to being a high pressure shower head is not what i expected. I have a water pic in my other bathroom and it puts out a much higher pressure sprays.
