Could I use this to draw a grid on paper (... to use as a guide) for art, for say a portrait or still life?

You can draw any square/grid on regular notepad paper up to 11" x 17". Not suitable if using a heavier paper than 28lb.

Is the back of the mat smooth and flat? (I want to use it as a pencil board.)

The back of the Freehand Designer is flat/smooth.

Is this product clear / see thru?

No, it is not clear. It is a solid white color. You are unable to see anything through it.

Will this work with 120gsm/80lb paper?

For the best results use 80gsm weight paper. If you use 120gsm paper, you will need to use a fine point pen and press quite hard when drawing a straight line.
