Will the ice pack stay on a person who is seated, if the strap is not used?

No, This product requires a strap to be used.

Why do you need to put in special Ziploc bag? And says to freeze flat but won't fit zip lock provided??

I keep it in the zipped bag when freezing because I don’t want it touching any other content in the freezer. I do not freeze it flat because it’s too hard when it’s frozen to wrap around my arm. Instead, when I’m done icing my elbow I loosen it up a bit and freeze it in the form of my arm. That way it’ll fit easily in the zipped bag. When it’s time to use it again I can just squeeze my arm through and then tighten it up.

Will it fit a 4'11 woman, 115 lbs?

Yes, however it is heavy. I bought it for recovery from shoulder surgery but I couldn't use it initially because of the weight. Once that initial pain improved, the shoulder pad helped greatly and it is a very high quality.
