H&P notebook - Medical History and Physical notebook, 100 medical templates with perforations

his H&P medical planner notebook has helped me tremendously during clinical rotations. Having a dedicated place to organize histories, physical exams, and assessments has made such a huge difference in how confident and efficient I feel on the wards. I just completed my first semester of clinicals with my PMHNP program and this H&P notebook was perfect for taking notes throughout the appointments. It was small enough and it did not distract from the patients experience.

It’s great! Has a lot of pages and the info is good to get a baseline when interviewing patients.

it is easy to follow but space to write is tiny given i normaly writy realy small. It is not 100 leaves, it is exactly 90 only. It takes a little while to get use to.

I originally got this for my boyfriend when he started 3rd year medical school rotations and he absolutely loved it. He said it is great especially for IM. It helped him organize his thoughts and notes and not forget to ask anything. He then got one for me and I agree! This is super helpful especially for medical students and interns, as they may need a little more guidance. It could also be helpful for senior residents and attendings, but less necessary since they are more experienced and may not need that level of guidance.

Love this book. Use it consistently throughout clinical rotations and it always reminds me of that one question that I inevitably forget to ask. Feels more personal than typing away in a computer in front of your patient and it’s small with very sturdy covers to carry around with you as you go in and out of rooms. Has everything you need and covers all of the ROS. great value. Also, has a sleek appearance that’s not too gaudy!!

Highly recommend this notebook for any medical student starting hospital rotations. As a third-year medical student, this notebook is an excellent guide for ensuring everything I need for an H&P is covered when I meet a patient. Not only are there plenty of pages, but they actually included space for labs and imaging results- something most of these notebooks omit. One of my residents actually asked to have the pages for a more complicated patient I saw because she was so impressed with the format. I was so pleased that the pages were perforated so I could easily hand her my notes- not something I actually thought I would care about! I never have reviewed items on Amazon before, but this notebook has been such a great purchase that I felt it was necessary.

I bought this notebook before starting my clinical rotations. It is helpful but has very small boxes to fill in for some of the history information. It also runs out very quickly and is slightly overpriced in my opinion. I bought it once to help take histories and now don't use it at all. Will not be buying another.

The book was covered in sand and then when I opened it up it was clear why… water damage 🤦🏼‍♀️

Was not what I was looking for. Might be terrific for others, but didn't meet my needs
