Is this case flexible material?

Only slightly, it’s not firm and plasticky and it’s also not completely foldable.

Can I use this case with an additional lens protector on? Or will it make it too bulky/not fit?

I would say no,. I don’t think it would fit properly over the camera lens

Does this have a built in screen protector or do I need to get one?

iPhones do not need a screen protector. The 13 has the strongest screen glass available. I have not used a screen protector on iPhones since iPhone 6. I have never had scratches on the screens. My wife still has a 6 with no protector and it still isn't scratched.
