I'm 5'6 and weigh about 240lbs will the 3x fit me?

The 10 to 11 size should fit you. I don't buy the high rise. I always buy the briefs.

Do these have a cotton crotch?

No it's not cotton, but special extra weaving that work even better and leave no gusset line.

Do the sizes run small?

Very hard to answer because sizing for women is all over the place. I'm 64, 5'4" and 148 lbs. I'm wearing a 10-11 very comfortably, and this size fit me just fine when I weighed 10 lbs more. I am a pear, so I'm more bottom heavy. I originally bought these in a store so I knew they fit, but like I said, they continue to fit at a lighter body weight, so they are forgiving. I love these panties--so soft, and slick enough that jeans, etc will slide over, not stick to, them. They do not ride up on me at all. Very comfortable.

Is the waist band snug rather than stretchy?

The waistband is just the right amount of snug. It is stretchy but it is snug enough that you don’t even know you’re wearing it. Best panties I’ve found!! So very comfy.
