Q: There are two bolt holes through the fabric on one side of the chair back, but not on the other - should they be on both sides?

They should be... I would send the chair back and get your money back if I'm honest. The chair is not worth the money or the hassle. I had a similar issue with the holes and had to cut the chair open to find the holes
