Q: Do resistors need to be used or will the load rely on the usb power supply and whats being powered by it?

If you want resistors in addition to whatever may or may not be in your power supply and/or device, you'll need your own - this is just direct connection from USB pin to PCB terminal. As long as there's a decent load you should be ok without external resistors; if you're just powering an LED or something then standard current-limiting resistor rules of thumb apply.

Q: Has anyony had issues with the usb connectors coming off the board? (microusb)

I have not but I do treat them as if they are fragile and sort of expect them to break. However, so far they have worked fine and without issue. I have lost a few but none have broken.

Q: If I input 5V, Will it output about 5 volts?

Yes, I have been using them to connect 5v to my train layout to power all the lights, they work fine.

Q: I assume these are fine for 12v applications?

Yes, as long as you use them with a 12v power supply.
