It turns front to back or left to right, no tilting

Does the stand need to be screwed into the counter or table top?

No it does not need to be screwed down. this would just increase the security of the stand . We move ours around quite a bit so that is why we did not screw it down

This was purchased for my workplace, and it worked wonderfully. We had two ipad pros out during an exhibition, and had no worries when walking away from the booth. We used the security cable to lock the kiosk to the table. We're looking forward to using it at all of our events.
