The pad is placed over the little toe and the cushioning flap is to lie over the painful lower joint. It is easily put on and positioned. The part that goes over and around the little toe can be a bit snug and slightly uncomfortable, whether from not being initially placed properly or later movement in my shoe I could not tell. My toe will become red and tingly sometimes causing me concern that it might be limiting circulation. However, I only wear it when going out of my house. The shielding pad does help considerably when walking, if properly placed, to protect the inflamed joint.
