Q: If the valve were pulled open and held below the level of the bag would the water pour out? Or does it require suction?

When I clean the bag, I open the valve let the hose hang below the bag ,the hose needs to be below where it connects onto the bag and give bag a little squeeze and let gravity do the rest. . So yes, if the valve were open below where it connects onto the bag it will run flow. So just remember to push in, turn valve close, and put cover back on.

Q: Will this fit standard size tube on most hydration packs (including Camelbak)? If not, is there an adapter that can be used to enable it to work?

Yes it will fit Camelbak and any of the rest of the the brand name hydration tubes.
