Q: what is the width of this strap

It is 2 1/2 inches wide and it is really comfortable and easier to adjust than I thought it would be. As I use it and work out a lot of the stiff ness in the new leather, it goes on and off the strap pegs on the guitar a lot easier.

Q: Would i be able to carve a design into it or is it too thin?

It's fairly thin. Don't know if you could carve into it.

Q: How do I put it on my guitar? Do I need to buy another accessory?

You'll need to have at least a rear strap button on the guitar to secure one end. Then you'll either need to have another button installed on the neck heel (the fat part of the neck closest to the body), or loop a leather shoe lace through one end of the strap and tie it to the headstock below the strings. The former method is best, and having a guitar tech or luthier install the button is very inexpensive.
