My upper arm circumference is 20 inches. What size should I buy?

Just use your blouse or shirt size. I’m normally a 3x and the 3x fit perfectly. My arms are huge. It fits close to the skin. Very comfortable.

Do they have it in Full length sleeves?

I searched for it and did not find one in full length sleeves. I also found that the middle section is a little short and wish that it came down longer at least pass the belly button. Hope this helps.

Does this run big or small. I have 14 inch shoulders and 40 inch underbust. I was going to order xxl, but I'm thinking xl now. Thank you for any help

Definitely I would go with the XL, I have a 38 1/2 underbust and I purchased size XL which turned out to be slightly loose on me . I reviewed that I wish it was tighter and longer. Hope that helps. Good luck!
