Q: What size hole saw does it require?

You need a 44mm hole saw. the ventilator needs a lozenge shaped hole not a round hole so you also need a hack saw and some other tools. Such as: , scissors, tape, drill, 2.5mm (1/8”) drill bit, 5.5mm (7/32”) drill bit, metal file, rust inhibitor and brush, 3mm (1/8") hex key, waterproof adhesive sealant.

Yes, that is one of the main reasons that people fit them.

I've not noticed any noise when driving or when pakes. It's really quiet! It's in the back cabin, separate from the seat cabin.

No. It extracts air, nothing comes in through the vent. You can also close the vent if you wish.
