Yes, it was $14 for one bottle. I bought it for my husband who remembered it as a child. No, I would not order it again, but he is enjoying it.

what is the expiration date?

The expiration date should be stamped on the outside top of the cap. For example, mine said, "Best if used by Apr 22, 2023"

Did this always come with cabbage and red bell peppers? I thought it used to just be pickles in with the mustard..

I'm not sure. I've never read the ingredients before, but I've used this product a long time (years) and I've never noticed a difference in taste. So yeah, maybe it has had cabbage and red bell peppers all along.
