Could this be used by someone with neuropathy, or would it cause pain?

Because of the varying symptoms and manifestations of neuropathy, that is hard to answer. My initial thought is that it would cause some pain because of the protrusions on the TheraFlow rollers. If there is any sensitivity at all on the bottoms of the feet I would not recommend it.

I would like to use this at the office. Is it effective when sitting down? I have plantar fasciitis.

Yes, you can! I usually have it under my desk and use it twice a day. I also take it with when I travel. Was diagnosed 6 months ago with plantar fasciitis (never heard of such a thing before), but do know that it is very painful, especially in the mornings. Started using it and i all i can say is that it works!!! I will be buying another one because i gave mine to my 88-year old father when i was visiting him. And it has also helped him with immensely with his foot pain when walking!

Can this help with knee/leg pain?

I suppose this would help as everything is connected. In addition, I use the 6" dia. high density foam roller that you can buy on amazon for my calves placing it under the knees, thighs, back and just rolling on it up and down under my body. Kinda like a "do it yourself" massage.

Is this flat or arched (angled)?

It is slightly arched consistent with foot contour.
