Does this arm cover have any cushion padding?

No it doesn't. I ordered black, it has a textured material on the outside but no padding on the inside.

How do the pads attach to the arm? I have a t-bar under the arm so I need one that slips on top, not going all the way around and attaching underneath

The pad slips on top and adjust to the arm's corners when you stretch the elastic of the pad cover. No strap and nothing underneath.

Can you use these on an antique wooden chair arms

My gut feel is is no. There is elastic around the boundaries that grabs the pad on the arm. They would need to grab on to something on the wooden chair to stay on.
