If all 4 inputs are 1080p resolution, connected to a 4K display, and mode is set to single image will the image be scaled to 4K for the display?

The scenario you're describing sounds the same as what I'm doing. Four 1080p inputs, with the multiviewer set to 4K output resolution. Despite the output resolution being 4K, single 1080p inputs DO NOT get rescaled to 4K resolution. What you see is a very large 1080p image (at least on my 65" 4K TV). It is a very clear picture, but you do not get the additional x4 screen real estate that you would if you connected the PC (set at 4K output) directly to the TV. Just a greatly enlarged 1080p image. Hope that answers your question.

Will this do 4k hdr or 4k sdr for the output?

I run mine on ultra HD 4K monitor resolution 3840×2160

Can this be ordered with a power adapter suitable for use in the United Kingdom?

Yes, you could order one fulfilled by seller, but it will be shipped from China and takes 2-3 weeks to arrive.
