Can this pan be used on an induction cooktop?

As others have said, T-Fal has some cookware that works with induction and some that does not. I bought this one T-fal C51782 ProGrade Titanium Nonstick Thermo-Spot Dishwasher Safe PFOA Free with Induction Base Saute Pan Jumbo Cooker Cookware, 5-Quart, Black and tested it when it arrived. It works well on my induction cook top.

From the top of the pan without the lid, it is 2 3/4 inches. From the top of the lid rim, it is 3 inches deep.

Does this work well on a ceramic flat top cooktop stove?

If it is a regular smooth top ceramic stove, yes. I've successfully used this pan on mine for years.
