According to the manufacturer's rep, the Magewell card does not capture 4k. The Magewell drivers are much better than the Blackmagic ones and can work with Direct Show applications.

Can the hdmi's be used as outputs, or are they only inputs?

Capture or input is only supported. Unfortunately, at this time Magewell does not offer an output device. This may change in the future.

Why is this so expensive? What does this do others dont?

My experience is that this one work perfectly. With others you may have problems to connect your HDMI camera. I did. This is not a waste of money.
