Q: Is that Gel directly under the foot or more of a hard orthotic type material?

Yes the Gel is directly under the foot - However if you place them in your shoe they shift and are not effective. We were able to live the shoe inner and place the Gel heel under there - this stopped it from shifting as you move.

Q: How do you stop these shifting when you put shoes on / walk please?

Personally not had a problem as long as you get correct size to fit shoes..also buy from cosyfeet too.

Q: How thick are they? How much height would they add to the user?

They are not too thick and don’t add any height they just cushion your heel

Q: Are the medium sized gel heel cups for ladies? And do they fit size 3?

I am a ladies size 6/7 and bought these to wear in my wellies which have an uncomfortable heel area. I think they may be a bit large for someone who is a size 3 to wear in court shoes but probably OK in low heeled walking shoes or boots
