I've read reviews for other mats that say that womens' heels (and not stilettos!) poked through the mat. Any issues with that?

Yes, the very small heel surface of my 2.5" pumps did puncture this mat. Stood on it without thinking and peppered one spot with punctures. HOWEVER, the cuts are only visible when the mat is flexed by hand to see them. They actually close right up and do not show or affect the mat's supportive performance.

Is this mat safe on/for hardwood floors?

Yes. However, it gets in the way of the chair when you need to sit and doesn't move without bending over to pick it up. For a foot movable option, get the SitStand SmartMat, which is made for adjustable height desks.
